On 1/7/my husband had symptoms of a stroke in the middle of the night, I called and AMR arrived, loaded him, and took him to the hospital (St.Dominic)A month or so later we received a bill for $The bill was sent to me, *** ***, and also listed me, *** ***, as the patient I called the number and told the person that answered that my husband, *** *** was the patient, I didn't even ride with him and followed in my carThey were going to refill the insurance and I was told to wait on a corrected billAnother month passed and we received the bill again, this time saying "late notice", but still not corrected and still listing me as patient We both called that week to re tell the correct info and we're told it would be fixedSince then I have received late notice after notice and phone callsStill, we would call but never get anywhereNow a letter from *** *** ***, saying the account has now been placed with *** *** ***

The billings are under my father He currently resides in a nursing home While residing in the nursing home he fell and broke a vertebrae in his back needing transport to the hospital and then to follow subsequent doctors appointments where AMR services were provided I have made calls to AMR at the *** to get them to bill medicare for these charges to no avail 9-6-18, 9-12-18, 10-9-18, 10-24- There are trip charges totaling over $1,for which I am just looking for them to submit to Medicare I have even had my fathers supplemental insurance call them on my behalf to get them to submit to Medicare Still to no avail On 9-12-and 10-9-and 10-24-I provided my fathers medicare and supplemental insurance information yet again to still no avail

Emergency Ambulance transport provided, I live in California, AMR rep Ritz says Senate Bill ABdoes not apply to Ambulance service and that they are allowed to Balance Bill me even though I have an HMO Insurance plan I have sent them a letter and spoke with them twice yet I received a ten day collection notice They are also billing miles for a transport that was only miles, I contacted them also about that and they again billed me for the remaining unpaid balance of using the base amount of miles

I am rejecting this response because: As explained to Ms*** on Friday 9/30/that is what previously happenedThe same information had been submitted before to the ambulance company, and a three conference call had also had been obtained with the insurance company (***) and I thought everything was fine however I receive a collection letter because the claim was never filedI requested that Ms*** follow up with the billing department of her company to make sure that the claim had indeed been filed and *** had received itI would also like this collection issue removed from my son's name and a statement issued (mailed out to my residence) that this was an error based on billing as this should not have been sent to collections in the first placeAnd by this company now stating that all they ever needed was indeed a copy of the military ID to process the claim which was supplied the date of transport I find hard to believeWhy weren't we contacted in the beginning for this instead of them sending the account to collections even after my husband's social security number was provided, my son's dob, social and benefit number? I am also requesting this is seen all the way through to completion.

4.Jan- My month old daughter was diagnosed with RSVWe were transported by AMR from one hospital to another (Total trip billed miles)We ended up staying the night at the hospital and released the next day1.May- We received our first bill from AMRYes that's correct almost months laterWe had two immediate issues with the bill#Wife's name spelled incorrectly
#Insurance company was incorrect
Being that the insurance company was incorrect inevitably meant the insurance information was incorrectThis bill arrives in the amount of $1,We immediately contact AMR and inform them of their mistakeMay- Our insurance company pays their max amount of $(After contacting insurance company was informed this is the standard amount paid for what was provided)8.May- We receive newly updated bill in the amount of $We again contact AMR for a detailed explanation of why these charges are so highThey explain to us they are not under contract with any insurance company, that they are a private company and can charge rates how they see fitAt this point we ask if there is any way to have the charges adjusted in order to have a bill that does not appear to be gouging a customerWe were immediately informed about their program to lower the bill if we did not make enough money (we would have to practically supply them with our taxes)We disagreed and requested that we negotiate and come to something we can both agree onWe were told there is no adjusting the prices, they are what they are, if I cant afford it they can help as long as I supply them all my financial infoAt this point we decided to point out how ridiculous their pricing wasCharge #- Base Rate $We asked what does the base rate coverWe were told it covers basic life support with the addition of oxygen, CPR, splinting, etc etcAt this point I asked why we were also being charge for the oxygen ($55.89) if it was covered under the base rateShe responded that oxygen was a separate chargeI then asked her to repeat what was covered under the base rate charge again, and again I was told "oxygen, CPR, splinting, basic life support needs"To which I asked again if oxygen was covered under the base rate why am I being charged for oxygen, to which she again replied, "if oxygen is used then it gets billed as a separate charge"At this point I could see we were going in a non-productive circleSo on to charge #Charge #- Mileage $Might I remind you we went miles!!!!! That's $15.53/miI began to point out I work on diesel vehicles and I am well aware of how egregious this pricing wasHere's the break down of how insulting this cost really isA Mid-Range engine in an ambulance type vehicle will get on the low end MPGThe mileage billed was miles, therefore gallons of fuelLet's round up diesel fuel to an even $3/galThat comes out to $Now it's not unheard of for a company to wrap a "usage" fee in to mileage or what might be called a "depreciation" costOn average a company will amount cents to cents per mile, so lets go on the high end againThat becomes $3.85/mi X gal = $that total cost comes out to cents LESS than what they charge PER MILE!!!!! At this point the lady decided to send me to Customer Advocacy and we agreedFast forward to today (14.Jun.2017) I called back to see if any progress had been made, I was informed non had been made but they will forward my dispute to Customer AdvocacyI'm pretty sure I'll get the same response I got last time they sent me to CA, which was noneSo for anyone looking in to using this company, I stress steer clearFind the OTHER ambulance waiting, I promise they wont be as bad as this company
My son was transported via this ambulatory service on 8/ It was not until 10/that I saw the claim process with AMR by UHC At this point we were awaiting to receive our bill an pay the amount owed By 11/had not received a bill but did start receiving calls from a group asking for personal information to speak with my about my son on a recorded line They did not identify themselves or the nature of their call On 11/I was able to contact AMR and track down that reason we had not received a bill was because they did not have our address Because they were OON I was advised that we would be paying the discount and owed more than my EOB stated I provided my address and asked for a bill before I would make payment I contacted UHC would advised they would be reprocessing the claim and would pay at a higher amount I attempted to notify AMR on this day but they were experiencing phone issues and could not be reached On 11/I received a VMail from someone that coul
Ambulance took a route away from the Riverside CA Kaiser hospital over country roads and non exxistent road on April This ride lasted about one hour but the direect route on Van Buren and #freewway (Miles) takes about minutes
American Medical Response did not do the correct paper work for a trip to the ER where my husband was admitted to ICUThey are suing us now in small claimsHe was in critical condition, but they are saying he could have driven himself? I consider it elder abuse and harassmentThank you
My wife has made numerous attempts for American Medical Response (AMR) to file their claim to our insurance companyI have documentation of the insurance card being faxed to AMR, the insurance card emailed to the collection agency (Kent Hollins Attorney at Law), and documentation from our insurance stating no claim was ever filledAMR representatives were told several times that our insurance will not accept the claim after months from the original date of serviceThe original date of service was on 2/19/AMR did not make the claim to my insurance therefore I should not be held responsible and the collection of the account should be voided
I received a precollection notice from AMR for Trip *** which supposedly occurred in *** on 5/16/ I don't live in ***, nor have I ever had a relationship or interaction with AMRAMR has never transported me and this trip was not meThey are attempting to fraudulently pursue me for a service I have never used, never contracted with, and have, until receipt of this letter never had any contact with whatsoever

We are elderly and disabled persons and have received numerous unsolicited and unwanted calls from this organization and number We answer and they state they are with American Medical Response I asked them for their name and they say "***" When I ask what they want they either hang up or tell us it is none of our business I then state "Well then, stop calling us or else I will report you to the Revdex.com for telephone harassment which I am doing I called the number about minutes ago and the people at the company says that no one calls anyone I tell them that I am calling the same number that appears on our caller ID every time they call
On 1/7/we needed an ambulance AMR *** responded On 1/19/17, we received a bill from AMR *** in the amount of $ I contacted my insurance and was told they had not received a bill I was then told that AMR was not supposed to send us a bill, however as they were outside of our providers, the insurance company could see the error I gave the insurance carrier all the information on the bill A month later, and subsequently every month thereafter, we received a notice from *** *** *** (who is apparently the billing company for AMR ***) indicating that they were working with our insurance for payment I contacted the insurance to find out if the bill had been paid On 4/27/17, I spoke with the insurance carrier and was told that they cannot pay the bill because *** refuses to authorize the insurance company to change the payment address from their POBox in ** to the *** ** address The representative from *** repeatedly talked over u

Provider billed my insurance company for service rendered in July of I paid the provider what was due Provider proceeded to take payment and apply to wrong account After that was fixed provider then sent me a for the the full amount billed again to my insurance company My insurance company said I didn’t owe any additional amount due to the negotiated rate they had with the provider They tried reaching out to said provider 3x with no action taken by the provider *** then told me they would proceed with fraud notification Since that time I have received another bill from the provider with the threat of sending me to collections in days After trying to call provider during business hours about the letter they wouldn’t take my call They had the phone transfer to a 3rd party service who said they were busy and were unable to take my call
So to summarize:
Provider is overbilling me according to my insurance provider
Provider isn’t taking call from insurance
