Per SIEA terms of service and warranty, a proof of purchase is required for all peripheral service.  Peripherals are eligible for service for one year after purchase; SIEA does not service a peripheral after 1 year from the purchase date.

I spoke to Dominic and his mother declined his request for a refund or...

forgiveness of debt.  All purchases were made from the same PlayStation and each purchase was emailed a confirmation.

Sony was able to deactivate the incorrect PS4 device from my account. This then allowed me access to reactivate the PS4 that I own.

As Mr. [redacted]'s PlayStation 4's warranty has...

expired, a fee for service is due. The fee for service includes FedEx shipping to recover his PlayStation; we use genuine certified PlayStation parts installed by qualified PlayStation technicians. We also cover the FedEx shipping back to him.
We cannot also extend the service warranty past 90 days.

We thank [redacted] for his feedback, but we will not waive the service fee.

this guy has yet to answer his phone or reply to my email.   He needs to respond fast I'm waiting.  Stop telling me to block bad people.   That doesn't solve the behavior problems that we suffer from these folks and Sony does a bad job of fixing the problems and so does [redacted].

As I keep stating the minecraft game has nothing to do with the service ticket. That was something a rep suggested on another concern. On the service ticket the representative stated that I will get a refund. - Monetization avenues remained open and accessible in the game when the servers could not handle game-play in which the monetized aspects were utilized. Inability to communicate a "purchase at your own risk" statement to warn users that purchasing secondary currency items will not guarantee you will be able to use the items due to the unstable servers. 
- Lack of response from a dev on forums, Twitter, email (after multiple emails were sent) regarding rank reset.  Reference: [redacted]
- Unhelpful responses on forum and locking comments to disallow responses: [redacted]
As I had purchased the MVP edition from a pre-order, monetized $70 in value, we're looking at roughly $140 invested in a product that does not work as advertised. On top of that, the customer service is lackluster and/or unhelpful. A counter-offer to accommodate would be acceptable. $30 in PSN store credit, perhaps.  
No resolution is unacceptable.
