Hi Mr [redacted] , I am sorry for the inconvenience that you experienced with your airfare with American Airlines though Costco Travel We are issuing you are refund of $to your credit card on file to resolve this situation Thank you for your patience and for the opportunity to come to a resolution

The member’s tile was ordered incorrectly. FloForm offered several different solutions. At first the member committed to a similar tile but the size was 3x6 instead of the 2x4, Floform ordered that tile, then member cancelled later that night stating that they wanted their original... choice. They were upset and stated that this tile is a standard and should be readily available, however this particular tile style does come from Italy and is not locally sourced. Floform's distributor was able to get their distribution to release the material much sooner than originally quoted. The member was scheduled for 5/25/16 install after the tile mix up and Floform was able to move them up to 5/13/16. Assistant buyer of the Countertop category talked to member today. She will follow up with the member after the 5/13/16 appointment to make sure he's satisfied.

Costco has settled this claim directly with the consumer

From what I am reading, the member was missing some information on her original submission Per the notes on her submission with [redacted] she was sent an unqualified letter asking for clarification Her 2nd attempt was still missing info The member was in communication with [redacted] on November 20th [redacted] has all the correct information now and the $check is being processed through the system

After doing some research under the provided names, membership numbers, and driver's license number we show in our computer system that [redacted] owes Costco $from two outstanding checks written to Costco[redacted] - [redacted] wrote check #$on 12/23/to the Santa Rosa CostcoIn addition there's a service fee of $that equals $We have an NSF cover sheet with communication with [redacted] on 1/20/that she sent a money order that should arrive 01/or 01/22/On 2/3/there's another entry we made that she stated, she's not sure why we didn't receive the money order and she will trace itThe check was sent to the D.A's office on 4/14/03.There's a second check written to warehouse #Woodland with a different membership number but the same driver's license number for [redacted] The check was written to Costco on 8/8/#in the amount of $plus a $service feeThe check was sent to the D.A's office on 8/8/

I have not received communication from the Bollingbrook store, nor was I aware I needed to respond as the last communication from Revdex.com said noAction was required at this timeAs the store has failed on multiple occasions to reissue the check and my address has not changedI did not renew my membership because they were unable or unwilling to issue the checkThe gift card will not do me any good as I cannot redeem it without being a member and I will not be renewing membership with a business that has taken almost a year to refund the $owedI want a check issued that I can actually use and that is sent to my mailing address they have on file________________________ [redacted]
