Western Dental Services, Inc("WDS") is in receipt of the Revdex.com's inquiry regarding this case. Please note that as indicated in our previous response to the Revdex.com, protecting member information is not only our company policy - federal law now requires it. That is why we
must first get a release of health information from our patient authorizing us to disclose any information regarding this case. WDS requires that patient complete and sign the "Western Dental Services Authorization Form" (see attached). As a result, WDS will responded directly to the patient regarding this matter. However, as soon as the signed release is received by WDS, we will forward any pertinent information to the Revdex.com

Western Dental Services, Inc("WDS") appreciates the opportunity to address the concerns filed by the patient with the Business Bureau. The Committee noted notes that there are inherent risks to dental treatments procedures that are accepted, and it is not always possible for the dentist to forsee or predict when or if a tooth may react adversely to a dental procedureAdditionally, no treatment outcomes can be guaranteed. Furthermore, the Committee cannot determine patient's current dental condition. Please note that following the placement of either new or replacement restorations, it is not uncommon for a tooth to be sensitive to changes in temperature and or bite. Patient is encouraged to return to the office for a re-evaluation and the determination of whether additional treatment is necessary. Patient was informed that due to the large deep existing fillings that were both decayed and fractured upon first presenting to the office, they had a potential for more treatment including the possibility of root canal therapy. Western Dental cannot verify the statement that patient was informed that the dentist drilled too deep thereby causing the need for root canal therapy on tooth #19. Patient is encouraged to provided supporting documents from subsequent dentistupon receipt of supporting documents a grievance appeal will be submitted on behalf of the patient Western Dental encourages patient to return to the office at no cost for a re-evaluation

The statement is so contradictoryMy insurance has not changed and has always stated to NEVER pay for fluoride treatments over the age of I spoke with my insurance company myself and has always denied that particular treatment for anyone over the age of A treatment that I did not need nor have EVER asked forIt is an underhanded way to get money out of patients. Again! MY INSURANCE HAS NEVER AGREED FOR COVERAGE ON FLUORIDE FOR ANYONE OVER THE AGE OF and Western Dental were well aware of this both timesI only owe for the X-Rays

Patient has paid a total of $2,405.00, leaving a remaining balance of $114.00. The itemized statement previously provided reflects all payments received. Patient is encouraged to review her paymentsIf there is a discrepancy in payments patient can contact, Connie Rocha at 714-571-to further assist in any missing payments. An audit has been completed on patient's account and confirmed that patient is not entitled a refund. Patient's insurance has paid at UCR (Unusual Customary Rate) fees. When patient contracted for treatment an estimate worksheet was provided and signed by patient. Estimate worksheet states that insurance benefits are estimated and actual insurance share benefit and patient share may be different. Based on insurance Explanation of Benefits patient's responsibility is $1922.00. As a good will gesture Western Dental will not charge patient $1,as insurance states. The over payment from the insurance company has been applied towards patient share. On 8/18/Western Dental modified patient share o to $Patient has paid a total of $leaving patient financially responsible with a remaining balance of $plus additional late fees and interests of $106.37 that have incurred on account. Patient is welcomed to contact her insurance company for confirmation of patient share responsibility
An itemized statement has been mailed to patient detailing all treatment rendered, insurance payments, and patient payments. Also, copies of insurance Explanation of Benefits have been provided to patient. Patient has been provided with a direct contact person, patient can contact the representative assigned to her account for further explanation of financial audit. Once patient contacts Western Dental representative and agrees to her financial responsibility of $226.25 the late fees and interest of $will be removed as a good will gesture

Per Grievance resolution letter dated May 16, patient was informed that the Committee
concluded that the diagnosis, treatment proposed and the orthodontic care provided was appropriate for patient's orthodontic condition. The Committee noted that patient's primary dental-facial-skeletal problem was the loss of nearly all of the mandibular (lower) molars. This led to several long-term problems and handicaps including the drifting and super-eruption of the maxillary (upper) molars. Without sufficient number of posterior (back) teeth to support the occlusion (bite), the bite collapsed and closed. This collapsed caused the forward/outward flaring of the anterior (front) teeth. Such highly compromised conditions are extremely difficult to correct with orthodontic treatment.
The Committee also noted the concern of the appearance of patient's anterior teeth. The Committee observed that the anterior (front) teeth are properly alignedWhile the upper anterior teeth are tilted very slightly forward, they are in harmony with patient's facial profile and patient's referenced dental-skeletal condition
The Committee noted that patient's upper and lower cuspids were placed in proper relationship in patient's dental arches. Accordingly, additional retraction of the cuspids and other teeth is not indicated and would not be on the best interest of the patient
Patient was encouraged to return to the WDS office in Manteca for re-evaluation to see if it is possible to make small amount of additional correction of the angulation of the anterior teeth. If necessary, any new records needed to complete this evaluation will be taken without any charge to the patient. Patient was also informed that in the event that the orthodontist determines that it is safe and possible to make minor corrections, WDS will provide an additional six months of orthodontic treatment, and new retainers (if needed) at no charge. Patient has not returned to Manteca office for a re-evaluation
Patient was also notified on her appeal letter dated July 23, that her concerns regarding the quality of orthodontic care, the Committee re-examined the history of patient's treatment. The Committee again concluded that the diagnosis, treatment proposed and the orthodontic care provided was appropriate for the patient's orthodontic condition. The Committee concluded that patient transfer request to another WDS orthodontic office is not indicated and will not be granted

Western Dental Services, Inc("WDS") appreciates the opportunity to address the concerns filed by the patient with the Business Bureau. Patient has been receiving billing statements for the incorrect patient. The system shows that patient's address and phone number were listed in
another chart that has the same name as the patient. The only similarity is the name, the account patient is receiving billing statements for has a different date of birth and different social security number. The mistake has been corrected and the collection agency has been notified to correct mailing address and phone number. Patient will no longer be receiving billings statements and phone calls for a different patientPatient credit not been affected since account is not being reported under her date of birth or social security number, therefore no compensation is warranted. Nonetheless, Western Dental sincerely apologizes for any inconvenience that this matter may have cause patientPatient was also personally contacted today for an apology and to reassure her that her credit has not been affected and that correction to the address and phone number have been made

Western Dental Services, Inc("WDS") appreciates the opportunity to address the concerns filed by the patient with the Business Bureau. When patient presented to the office for a consultation on 3/13/exam and a panographic film (x-rays) we completed. Upon completion of exam it was
determined that patients' blood pressure was high, the dentist then determined that treatment cannot be started until a medical release form was completed by patients' primary physician, please see attached medical clearance form provided to the patient. The office inadvertently opened a contract based on the diagnosis that was provided by the dentist. When the patient contacted WDS to address her concerns the contract was immediately closed. At that time patient was informed that she was responsible for the WDS plan sale of $that covers exam and x-rays. Please see attached Patient Information Sheet signed by patient. Patient consented and agreed to treatment and acknowledged consent to the examination and/or treatment by doctors, doctors' assistance and other medical personnel and is financially responsible for all charges for services performed by provider. Also, see attached Health History form completed and signed by patient. Patient consented to the performance of x-rays and exam, therefore patient will remaining financially responsible for the WDS plan sale of $to cover services performed. If a plan sale is not purchased UCR (Unusual Customary Rates) can apply. Without the WDS plan sale the fee for the exam is $and $for panographic film (x-ray). Patient is encouraged to contact our payment department at 866-620-Extfor a payment arrangementAlso, the credit bureaus has been notified that a contract was opened by mistake and credit reporting will be updated to only reflect a balance of $79.00. Once the $is paid, the credit bureaus will be then notified again that patient has a zero balance. Western Dental sincerely apologizes for any dissatisfaction and inconvenience that this matter may have caused patient
