customer signed up for their free trial on 04/23/2016. Attached is a screenshot of the terms when signing up for the free trial. Also, the terms and conditions are on the invoice of the initial shipment along with the next ship date. This customer contacted ProbioSlim customer service center on 05/02/2016, an additional free trial was shipped and the trial period was extended. On 05/07/2016, the customer contacted ProbioSlim customer service center and the trial period was extended further to July 1, 2016. The customer was then billed for their monthly supply on 07/08/2016, in the amount of $39.99. This customer contacted ProbioSlim customer service center on 07/08/2016 to cancel the account. To date, no return has been received by our Returns Department. On 07/08/2016, we issued a refund in the amount of $39.99, as a good faith gesture to our customer. We apologize for any inconvenience this process has caused the customer. We trust that our actions satisfy both the customer and the Revdex.com and consider the issue closed. The customer was then billed for their first monthly product on 04/04/2016 in the amount of $74.98 ($69.99 plus $4.99 shipping and handling). On 04/04/2016, the customer has contacted the Peak Life customer service center to cancel their account and obtained return instructions. To date, no returns have been received by our Returns Department. On 04/04/2016, we issued a refund in the amount of $69.99 and on 04/18/2016, we issued an additional refund in the amount of $4.99 bringing the total amount refunded of $74.98, as a good faith gesture to our customer. The shelf life of our products are 2 years from when they're shipped. We apologize for any inconvenience the customer may have suffered. when I called to cancel the account I asked for information to return the product they claimed was enroute to me, I had not received it. their customer service representative said they do not accept returns. I indicated then I would refuse the product as I did not order it - I never received an email claiming it was being shipped. They are deceptive and left me no recourse but to refuse the product. They have still not resolved that with my bank.

customer’s bank or credit card issuer. The customer signed up for three free trials. Two were ordered on 06/20/2016 and one was ordered on 07/01/2016. The customer was then billed for their first monthly products on all three orders on 08/31/2016 in the amount of $80.58 ($69.99 plus $4.99 shipping and handling plus $5.60 tax), for a total amount of $241.74. On 08/31/2016, the customer contacted the ProbioSlim customer service center to cancel all three of their accounts and obtain return instructions. On 09/01/2016, we processed a refund in the amount of $80.58. On 09/12/2016, we received all three shipments back at our Returns Department and two more additional refunds were processed in the total amount of $151.18. On 09/14/2016 an additional refund was issued in the amount of $9.98, bringing the total amount of refund to $241.74. We apologize for any inconvenience the customer may have suffered. customer’s bank or credit card issuer. According to our records, on 09/12/2016, the customer placed an order over the phone for a monthly supply trial of ProbioSlim through our customer care center. Per the recorded phone conversation, the customer was read the terms and conditions of the trial, was informed of the recurring conditions, in 34 days, the bottle will be discounted at $39.99 plus shipping and handling plus applicable tax and to call us back and cancel if she no longer and we have verbal confirmation that the customer agreed. A confirmation email of the order was sent to the email address provided. On 10/16/2016, the customer was shipped a monthly supply for $45.99 ($39.99 plus $2.99 shipping and handling plus $3.01 tax). On 10/17/2016, the customer contacted the ProbioSlim customer service center to cancel their order and obtain return instructions. To date, no returns have been received at our returns department. On 10/31/2016 we issued a refund in the amount of $45.99 as a good faith gesture. We apologize for any inconvenience the customer may have suffered. the customer contacted ProbioSlim Customer Service Center to cancel the account
and obtained return instructions. To date,
the trial has been received by our Returns Department but not the monthly
supply. On 05/16/2016, we issued
the refund for the disputed amount of $74.98, as a good faith gesture to our
customer.
