The company did refund my money. However I disagree that it was highly noticeable on their website that they stated they were not affiliated with the US SSI Office. I don't know what the site looks like now but at the time that I used it - there was no evidence of that. I can read - I have a bachelor n master's degree from Tulane University. If they have made changes to their website so that the public is immediately informed that they are not affiliated with the Federal Government and that they charge a fee for their services - then I am satisfied.
I thank you for your service and your quick action! I was mortified to find that I had given such personally identifiable information to such a website! I pray they delete the information as they claim.

e-forms.us represents themselves on the internet as part of the Social Security Admin.gov and asks for a credit card number, charging you $38.00
e-forms.us represents themselves on the internet website as a part of the US Social Security Admin. and asks for your credit card info, then charges you $38.00 for forms you can download for FREE from the .gov website. I thought I was dealing with the Social Security Admin. and was trying to obtain a replacement Social Security card, and all I received from this company was the forms that can be printed out for FREE from the .gov website. Their website indicates that they are processing your application for a Soc. Sec. replacement card. I talked to the Social Security Admin, and they said they knew about this website, which they said was a "scam". Also the phone number they gave AMEX when they sent in the charge on my card, is NOT a working number.
This company has no actual product or service offering and their website should be shut down and they should be barred from continuing operations.

It's ludicrous that E-Forms.US claims to provide assistance in filling out the SSA form for a replacement Social Security card, since all of the fields in the SS-5 form are only pertinent to the person filing out the form and no information could be provided or supplied by E-Forms.us. All you obtain for the $38.00 charge are the forms that you can downloaded yourself from the .gov website. They know that the mailing label they supply is bogus, since you have to show up in person at any Social Security office, since the SSA requires you to present in person either your own driver's license or an active passport. When I talked to the Social Security Admin. about E-Forms.US and told them what a scam it was, they said they were aware of the company and had many people complain of being scammed. Their $38.00 charge is so small because they know most people won't pursue a refund or file a complaint with the Revdex.com. This is how scams work and I'm certain they're taking advantage of many people who really can't afford the $38.00.

This is not something we made up, it is coming directly from their site. If you continue on the page you will also see option 3 that states: "Take or Mail this information to us." They give the option to the person to go to the office or to mail in their information.

The document that *** received was a 7 page attachment with the pre-filled SS5 form, the shipping label, and instructions; not spam. The customer requested a cancellation and the transaction was cancelled the same day of purchase.

Mr. was issued a partial refund of 20.95, Feb 26, 2018 as a courtesy because we have a 3-day policy. Full refunds are issued within 3 business days if the customer decides not to use the service. Amount of $17.05 was refunded 2/28/18.

Scam, false advertising, deceiving.
I provided private information under the impression it was a safe site. Very disappointed and worried about my identity. I purchased a "Social Security Package" on 2/8/18 for $38. This website is the first result on a Google search for Social Security Care Replacement. It was deceiving, and appeared to be affiliated with Social Security. It asked for a Social Security number, amongst other private information. The site was secure.
Termination of business for deception. Be unable to advertise at the top of the Google search. Encourage Social Security to warn the public of this scam.

Consumer mistakenly completed request for Social Security #s from a private company rather than socialsecurity.o.
Consumer has average computer skills and mistakenly completed form for a child's social security number request via e-forms company rather than social security. org website ~ 2 weeks ago. E-forms was provided both parents social security #s. Once E-forms were received via email, consumer realized the company was not social security.org and form was incorrectly completed. To troubleshoot form, adequate instruction was not provided on the website for correction. Consumer was required to goto social security website and troubleshoot. Consumer called e-forms for support and did not receive satisfactory answer. Consumer completed comment with company that they were dissatisfied with service and could receive a full refund but it is the principle. The consumer is disturbed that social security numbers were shared with E-forms.

online name change presented itself as an online process, but was just a form they would send you.
I searched for social security name change so that I could see what the process was. I got a search result for "online name change". I progressed to follow the process thinking I was doing an online name change with the ss department. turns out I was entering private information into a site that was not the social security site. I never got a form. details of the process are not on the site until after you compete entering the information. I am not afraid of identiy theft. I feel silly that I fell for this. but I don't want it to happen to anyone else.
this buisiness should not be allowed to advertise online name change when that is not what they are doing and online name change is not even alowed by the social security administration

I want a refund for $38. The company does not issue a social security card. All I received were forms to send in for one
I need a social security card...went online and the search shows that they offer replacements.. I completed the info.. they charged $38...once submitted they emailed me paperwork to mail off for one
