April15, 2016UPLOADED TO Revdex.com.org website Revdex.com Phoenix, ArizonaRE: Complaint ID# [redacted] ("Complaint") To Whom It May Concern:This letter shall serve as a response on behalf of Matrix Absence Management, Inc("Matrix")to the above-referenced Complaint filed by [redacted] '.Matrix is the third-party administrator for the short-term disability benefits plan offered to employees of [redacted] (the " [redacted] STD Plan" or the "Plan") This means that Matrix is charged with handling requests for benefits under the plans, including ensuring that employee/claimants submit required medical and other support for their initial eligibility for benefits.Our records show that on or about October 7, 2015, Ms [redacted] filed a claim to extend the benefits she was receiving since May under the [redacted] STD Plan Shortly, and on many occasions, thereafter, representatives of Matrix informed Ms [redacted] that, in order to evaluate her claim for continued benefits under the Plan, we would need to receive supporting medical documentation and that, while Matrix would endeavor to request those records on her behalf, it was ultimately her responsibility as the claimant to provide the necessary supportOur records show Matrix also made three separate attempts to contact the physician we understood to be her treating physician to obtain the required medical certification, without success.After not receiving the required supporting objective medical certification to justify benefits under the Plan beyond October 6, 2015, Matrix denied Ms [redacted] 's claim and sent her a letter dated November 12, 2015, explaining the bases for the denial decision in great detail and providing her with notice of her appeal rights under the PlanThis is the avenue for a claimant under the Plan to request reconsideration of a decision As of this date, she has not done so.We trust that this information resolves this Complaint as to Matrix
the FMLA was approved but the approval date says a start day of July and it was actually needed January 1, This issue is not resolvedI notified my case manager of the date discrepancy and she did not respond
After reviewing the response submitted by Ms [redacted] , it is unclear what additional response she or the Revdex.com is seeking While the parties appear to continue to disagree about how and whether the records Matrix received were timely submitted or received to support her claim (please see initial response submitted by Matrix for more details on our position), she indicates she is "happy" because she received the STD benefits she was seeking and is back at work, so we see no further need to respond or any additional complaint lodged that requires us to do soSincerely, Gail C***, Director of Employment Law & Compliance

The Claimant has recieved payment per the terms of his employer's policy. His perception of delays were the result of standard elimination periods and payment of benefits in arrears. Responses to the Claimant's phone inquiries were made promptly, but were directed to his voice message

Matrix never sent me a letter back in august. Per matrix representative Michelle G*** she informed me that a letter was created but never sent by Darryl T***. Michelle sent me the letter that Darryl was suppose to send me in April 2017. There was
a mistake made on matrix end by Darryl T*** who is no longer with the company. The only thing Matrix has to do is say a mistake was made in error and rectify the issue. What's worse is that matrix stated that there was no corrospondences from me to matrix but I have email between me and Darryl up until December of 2016. Matrix reps state that they do not see any emails in there system but I have them saved in my email. Matrix is trying to avoid there responsibility/ contractual obligation to provide service for a disable person
The paperwork was faxed multiple times from the HR officeSaid paperwork was not found until this complaint was filedNow that it is found I'm happy, I can return to workThis isn't the first time this is been a issue, the fax machine only seems to work when I bring in a third partyAlso I asked times before I had to go out for the last surgery for the paperwork to be faxed to my doctor and to give me an updateNether was doneSo I'm back in work again, that will do
I never received a letter from matrixI got an email from donna ri*** stating what disability meant. the paper I am including with this is my return to work paperWhy would they have me going back to work that date if I could go back sooner. I had my operation on July 19th, it takes about monthes for healing
I am not happy with this company or how it has handled my claim from the beginningTheir letter of denial was complete of inconsistencies, and they even contradicted themselvesThis company has a reputation for being sued for denying benefits to claimants on Incorrect information. They also made a "threat" to disclose their review to SSD which was also in review, trying to scare meI was physically examined by a physician assigned by SSD and my benefits will continue indefinitely with periodic reviews. Matrix however, used a "consultant" with no expertise in my area of concern, and he made a decision based on information provided by MatrixHis comments were completely irrelevant to my case, so either he overlooked the pertinent necessary information, or Matrix neglected to provide him the necessary paperworkHe never interviewed me, never examined me, and formed his own opinionThis "consultant" is also known for being sued for finding in favor of the company and denying claimants their benefits.Once I file my appeal, I expect a quick response in favor of continuing my benefits, and if not I will be suing them in courtI'm sick and shouldn't have to be going through this difficult time fighting for my rightsI want to be working but am not in any state to do so right nowI also want to prevent this from happening to other claimants in their time of need as Matrix has been sued over times in the last years and lost because they run a sham company, have no morals, high turnovers, and former employees even complain what a horrible company to work for
