I have spent several hours on phone calls with Anthem to resolve this and sent several email regarding this through . This is an unacceptable response. Please call me to resolve.Attached is the transaction history of what was debited and credited from my bank account. I replied to a message through Anthem's website on 10/20/16 submitting the bank information they requested when they responded to my request and have not heard back from them.
