[redacted] signed a contract on 4/27/ agreeing to an automatic payment of $39.00/month continuing for months unless we received written day notice to stop the payment She never contacted me until January 11th of stating that she thought her membership was on "hold" When I asked her why she said she just had not been coming to Curves We only put a membership on "hold" for medical purposes with a written note from a doctor She never contacted us about putting her membership on hold for any reasonI have attached the documents you will need There are of them

issues so that she can continue with her strength trainingI reminded her that she came to Curves for health reasons and we want to support her wellness goals and get her back to her strength training.Ms [redacted] became verbally abusive on a personal level towards me and uttered threats about damaging our business reputation via media, [redacted] and complaints to the Revdex.com if we would not let her out of her contract.Due to the escalating abusive comments being made by Ms [redacted] I respectfully let her know that I was going to end the conversation and hang up the phoneI then hung up the call and followed up with an email to Ms [redacted] (See attached email)Incident Regarding Use of the R-Word:Ms [redacted] was working out our facilityShe was being coached on the correct form on the squat machine when she uttered a statement using the word 'rded' in an unacceptable way`Ughh, I'm so rded, it takes me a while to get it.' I believe that Ms [redacted] was unaware that the member working out on the machine next to her was cognitively delayed and the member two machines over has a daughter who is cognitively delayed.As her coach on the circuit that day, in a quiet voice I (Tammy G***) said to Ms [redacted] - 'that is a word we don't use like that as people can find it offensive'Ms [redacted] 's further comments indicate she was aware of the word that I was referring toAs she moved to the recovery station mat next tothe squat machine, she continued to explain that she uses this word all the time, that she didn't mean to be offensiveHer voice volume increased as she explained this.Due to her volume and repetitive use of the word in her explanation, I noticed the other members starting to pay more attention to Ms [redacted] commentsSo I added more information for Ms [redacted] on the negative impacts of using that word in the context she was using itI said that perhaps she had heard about the yellow card programA couple of summers ago there was an event with Special Olympics and the Police held in Churchill Square to bring awareness to the yellow card programThe yellow card program is the ability to call a 'yellow card' on someone who is using the R-wordYellow carding a person allowsthe opportunity to help them become more? socially aware of the impact of the use of this word on othersAt the time Ms [redacted] did not appear to be overly upset by the discussion.As to the timing of the discussion, our circuit takes a total of minutes, with members changingstations every secondsOur conversation was not a continuous amount of time, it was interspersed with coaching other membersI recall finishing the end of the conversation with Ms [redacted] on the Ab/Back machine which would indicate that the entire conversation may have lasted a maximum of 7minutesAt no time did I raise my voice to Ms Coulesan during the discussion or make comments about her on a personal levelI provided her with information to increase her awareness.Desired Settlement:Our first concern remains Ms [redacted] 's wellness goalsOur first recommendation is to proceed with the offer extended to Ms [redacted] to arrange a facilitated meeting so she may address the personal issues and restart her work on her strength training goalsAs an additional good will gesture, if Ms [redacted] participates in the restorative process, we would be willing to waive Ms [redacted] 's May fee of $to compensate her for the period from April 2nd to May 2nd when Ms [redacted] did not attend the facility.Alternate options continue to include transfer of her membership to another club, continuing monthly payments to the end of contract, or a lump sum cancellation payment as per the attached email dated May 2nd, 2017.It is our sincere desire to find an amicable resolution with Ms [redacted] .Our position regarding the personal issues surrounding the complaint are that we respect Ms [redacted] 's right to hold an opinion and a different viewpoint on the use of the R-word.In addition, it should be noted that we do not feel that Ms [redacted] was being intentionally offensive or abusive to those with special needs working out at our clubWe believe her actions stem from lack of knowledge and understanding regarding the impact of her conduct.The club stands by our decision not to allow the use of the word 'rd' or 'rded' in the context that Ms [redacted] wishes to use it at the club [redacted] We were not singling out Ms [redacted] in correction for the use of offensive wordsWe do not permit the use of several words that do not fit our inclusive culture at Curveseg [redacted] ***, etc.Any club member using words like these in an offensive way are kindly corrected by making them aware that their figure of speech is landing on other members in an offensive way.While we continue to seek an amicable solution, based on Ms [redacted] 's statements to us and the Revdex.com, we feel compelled to add the following information for Ms [redacted] 's consideration[redacted] ***Thank you for your time and consideration of this materialPlease do not hesitate to contact us if you require additional information.Sincerely,Tammy G [redacted] OwnerCurves"Curves Terra Casa(780) 484-4234(780) 484-(FAX)CurvesEdmontonWestettamaii.com? I feel that they have refused to provide the service contracted for but have taken the money for those services
I was completely unhappy with the services provided, repeatedly asked to talk to the manager, but they refused to call backI fulfilled the terms of the contract, canceled the contract and they refused to give me gift certificates for the un used portion or refund the unused portionAfter repeated calls to try and resolve the issue via phone calls, they have refused to return any calls
I received your complaint from *** *** on August 9,I am writing in response to her claims that

I am in possession of a contract signed by Ms*** on January 4,I am also
in possession of a
signed “Member Auto-Pay Authorization” signed on the same date authorizing Curves to deduct the
money from her checking accountI have included blank copies of these documentsLast week, I
forwarded copies of the original documents to Ms*** for her review
In her dispute she states she joined Curves in February of while she actually signed the contract on
January 4,She worked out only two times at our club on January 8th and on January 30thWe
periodically make phones calls to members who are not currently working out and we made such calls
to her on February 26th and again on April 9thDuring the April 9th conversation, while only
documented as being contacted, I remember speaking to her at which time she told me she was not
impressed with Curves and wanted to cancelI told her that she did not give the club a chance and
encouraged her to come give it another tryShe said that she would try to make it in on the following
Saturday morning but never showed upThat is the last contact I or anybody else at the club has had
with Ms***
While our contract does not actually give a cancellation clause for reasons other than medical needs
documented by a signed note from a physician, moving away from the area serviceable by a Curves or if
we go out of business, we do allow people to cancel with a written notice and a fee determined by their
length of time at our club with a maximum of $Not using the facility is not an allowable reason to
cancel the contract
She claims that our club is too small and our staff “follows you around pushing you to do more than you
are able”Our club is designed as per Curves specifications and is a very appropriate size for what we
doOur staff is trained in encouraging members to work to their potential and what sets Curves apart
from other gyms is our “coaching of the circuit”
She also mentions that we are opened inconvenient hoursWe are opened 7:am to 6:daily while
closing for lunch from 1:to 2:The hours are posted at our club and papers are available for
members to take home with the club hoursIt was my understanding when she joined, that she did not
work and that she was flexible as to when she could come
As for her claim that! am continually billing the four other members without out their permission, that
too if falseOne of the ladies is actually a “wellness” member and has never been billedThe other
ladies have been contacted about their lack of activity here at the club and I have never received a
request from either of those requesting cancellationThe other two ladies have actually used the facility
several more times than Ms***
I received a note on August 9th from Ms*** to cancel her contractI will process the cancellation
request but will be billing her for the $cancellation fee that must be paid or it will possibly be sent to
our collection facility

Ive been struggling with back acne yearsIve tried chemical peels and microderm in the past so I decided to look into getting a back facialAfter researching online I saw that ME offered anti acne back facialsI made an appt for May 20th The manager, Mark, did my consultation and checked me out afterwardsI raved about how awesome my technician wasHe then went on to tell me about the membership and its benefitsThis would be my first time ever doing a membership anywhere so I was sure to ask questionsI left feeling really excited about the membership and I even told my friends, family, and coworkersMy understanding was that the membership was month to month meaning if I didnt want any services one month I could cancel that month and resume the next monthThe 12mth agreement was not thoroughly explained
My second appt was a massage on June 12th I was unsatisfied with the serviceI
Christopher Construction does not care if the end result is to the customers pleasureAll he wants is his money and does not care beyond that This is besides below average work on top of that

I am writing in response to the letter we had received and the complaint made against American Heating and CoolingTo respond to the issue To the comment he had made about wanting the $service call, we do not have a service call at that priceWe have a clean and check on a heat pump at
that priceHowever, Mr*** was having a problem with his heat pump when he had called inThe complaint about his unit was that it was low on Freon and that it does this every yearIf the work had been done by a good contractor the first year the problem would have been taken care ofThe minimum cost for that would have been $($trip charge and $diagnostic fee)This is if they do not decide to go with a repairHowever, if they do decide to go with the repair, we waive the diagnostic fee and then it would be the cost of the repair and the trip chargeWe are not a time and material based company, we are flat rateEnclosed you will find a copy of the original invoice and the work that was done to try to solve the issue with the heat pumpAlso a comment was made by Mr*** to our Service Tech to the fact that four years ago he had called us out to give him an estimate for a new heat pump and that we were $to $higher than the other company that he had contactedWe were not sure if that was the case, why he called us out to check out his heat pumpAccording to our Service Tech he was trying to save him the cost of a new unitIf you have any questions, please feel free to contact me.Sincerely,Murrell DL***Owner
Massage Envy has been charging my credit card for the past eight monthsI have only received one service at this location
