I have replied in writing the details of my appeal and sent the document I was provided which clearly states dependent coverage is until age (my daughter was at the time of her death) and till age if a full time student It does not say till the dependents 23rd birthday I was provided a deadline of September *** and the document arrived on September *** I was also asked to call to let them know my response was be submitted which I did and left my phone number I have yet to receive a response They stated in the letter they had attempted to reach me by phone twice and left voice messages which I did not return I received no calls or messages on my personal cell or my work number

I have not received any calls from Guardian and Guardian Insurance has not been able to resolve this case since 5/*/After further investigation with my *** ***, Guardian has denied the 2/**/claim for code *** stating, "*** *** *** ***" and no further explanation despite dental imaging (provided to the insurance) showing >50% canal obstruction, proving medical necessity, and a typed letter from the provider We have mailed appeals to the address Guardian provided and Guardian claims they have not received them and will not provide me with a phone number to contact the appeals departmentWe feel Guardian has been delaying this case so the claim would fall under the "*** *** *** ***."

I have not asked Guardian to pay for claimsI have clearly asked Guardian to process the claims accordinglyI am fully aware that the dentist I have chosen to go to DrD*** is not a contracted dentist, however I have chosen to go thereI am asking for an EOB showing denial so that I may pursue other means of paying for the dental work that is much needed for months nowDue to the delay in processing my claims I am suffering at the hand of GuardianI plan on dropping their services and contacting my local news so that other consumers/patients are aware of the agony that Guardian has put me through as I wait an unwarranted time for claims to be processedProcessing a claim does not only involve paying, but denialsI am asking that I received updated EOBs with denials in a timely fashionI thought I was clear in my request and complaint, but it seems as though my words have been misconstruedThank you Revdex.com for your help.
