Fitness cancellation policy and Annual Ratelock FeeThese documents were signed at the initial point of sale and her agreement begin date of May 15, Both the membership agreement and cancellation letter were sent to a Revdex.com representative via email ( [redacted] ***) in [redacted] has stated she did not use the facility during that time but there were no reasons related to YEP! Fitness that prevented her complete gym access, 7-days a week during the life of her membership
