issuesPatient being taken to a therapistConcern for tracking The result was the mother scheduled her son for a full new patient evaluation. On 2/23/17 the mother states [redacted] vision referred her son “about some signs of vision issues.” She further states on paperwork she has noticed and is concerned about “lazy eye in the past and behavior issues.” During examination, the mother states her son was referred because the previous optometrist noted a tracking issue. Mom states he is “clingy and not graceful.” He gets red rings around his eyes. Also, mother states he gets angry at home because he gets tired at the end of the day. His behavior issues include not wanting to listen and throwing fits. Lazy eye exists in family on both sides (mom & dad sides). Testing revealed the following diagnoses: Strabismus, esotropia, and exotropia. (eye turn in and eye turn out)Also, Hypertropia alternating (vertical misplacement of the eyes)Poor fixation or stopping to look at an objectOculomotor dysfunction—saccadic eye movements, pursuits dysfuction (tracking and locating for reading)Suppression of one eye during binocular (2 eyed) coordinationBinocular Dysfunctio (2 eyed)Severely restricted visual attention field of 9 degrees right eye and 2 degrees left eye (normal is minimum 70 degrees each eye)Visual motor skill integration dysfunctionVisual spatial dysfunctionVisual discrimination dysfunctionVisual sequential memory dysfunctionVisual figure ground dysfunctionVisual closure dysfunction Vision therapy was recommended to remediate the areas vision dysfunction and visual diagnoses. The family chose not to proceed. It is not our policy to refund money from 5 hours of testing (total cost $701.00) just because a family chooses not to proceed with the necessary treatment.
