was a miscommunication. Dr. B[redacted] again reviewed his plan of treatment for [redacted] today.| reviewed the ledger of accounts with the patient and have attached them for you review.1. Patient was seen on August 18, 2016 Appointment was requested for acleaning and a consult for an implant and scheduled. The Patient [redacted] stated that he thought he had [redacted] under his wife's plan. He was seen by our hygienist and a full new patient exam was done. The patient was then seen by Dr. B[redacted] and a consultation was done at no charge to the patient however due to the patient's dental history Dr. B[redacted] requested and explained in detail that he would need a CT scan done to evaluate his bone capacity before proceeding with treatment. The procedure (CT scan) is done on all implant patients. Patient agreed. (See attached Doctors notes). The CT scan was performed on 8/18/2016.
