We have changed our policy of billing so this will not happen again as stated in the letter. It is a requirement of most employer to have the social security number listed on the bill for the Occupational Health Services. The social security number is an identifier so when we send the bill to the employer requesting the service they know which employee received the service and who they are paying the bill on. This was not an open disclosure of [redacted] social security number. [redacted] employer has a contract with our clinic for confidential services and Gale provided his social security number in the past on the Company account and his social security number was linked as an identifier on that account. Per our conversation with Gale we have removed his social security number from this account. His company requested the service and we bill the employer per our contract. In our conversation with Gale he stated he works at the company and opened the billing statement we had sent to them.

Dr. Johnson, a Family Practice physician, referred Ms. [redacted] to Dr. Thomas, an Ear, Nose and Throat specialist for her condition on December 2, 2013. On January 3, 2014 Ms. [redacted] had an appointment with Dr. Thomas. January 8, 2014 Ms. [redacted] called in to our Operations Manager with her concerns about her visit with Dr. Thomas and she notified Ms. [redacted] of Dr. Johnson’s retirement. The Operations Manager submitted Ms. [redacted]’s concern to our Medical Director who thoroughly investigated this using our peer review process which included chart review, and discussing the case with Dr. Johnson and Dr. Thomas. Dr. Johnson also called Ms. [redacted] on March 20, 2014 who explained why he had referred her to Dr. Thomas and this was appropriate. Our Medical Director sent a letter to Ms. [redacted] explaining that the clinic had investigated her concern and the care she received was found to meet the standards and the charge will stand.
