Review: Contacted Dental Office in August 2015 to make an appointment for a tooth ache, left 2 messages and never received a phone call back. Actually went down (early Sept. 2015) to address of dental office and they had moved OUT with no note of a forwarding address. I am concerned about my dental records and personal information and how to retrieve them.Desired Settlement: Where did they go?
