Overpayment check issued more than 60 days after required by Federal Mandate per OIG. Patient deceased prior to receiving refund. Payment was made by daughter who is legally entitled according to Intestate Succession Law. Upon several requests to be contacted by manager (***) to no avail further steps are having to be taken.

Other (requires explanation) Overpayment check amount refunded in patient's name was paid by daughter via credit card and has requested that a check be re-issued in her name. All requests have been denied. As there was no will in place, the company is refusing to send a refund.
As sent in a recent email to Revdex.com this complaint has been resolved with the company to my satisfaction.
Verity Health sent to many users a letter on February 6, 2017 that on January 6, 2017 the organized learned that some of my personal information may have been accessed by an unauthorized third party. This includes patient names, dates of birth, medical record numbers, addresses, email address, phone numbers, and the last four digits of credit card numbers. Given this, they have offered Equifax to provide credit monitoring services to me. I called their hotline number at XXX-XXX-XXXX on February 8, 2017 and was informed that it was to expire from the dated letter (February 6, 2017) which would result on May 6, 2017. When I attempted to redeem my activation code to enable my Equifax credit monitoring services on May 5, 2017, Equifax informed me that the code expired. I called Equifax on May 8, 2017 since the hotline was available Monday to Friday from 6AM to 6PM Pacific Time only. I informed *** of the situation and she escalated the situation. *** called me back On Tuesday, May 9, 2017 and stated that Verity Health is unable to honor my request. After I told *** that I felt this was unjust given the phone call from February 8, 2017 that it was to expire 3 months afterwards, *** escalated the situation again. On Tuesday, May 9, 2017, *** informed me that Verity Health will not honor my request.

Other (requires explanation) I would like a replacement for the Equifax credit monitoring service that was promised.
