Valley Health Berkeley Family Medicine & Associates

On October 17, 2017 I was a trauma patient at Berkeley Medical ER located in west Virginia. I was asked for my social security # by a staff member present in the ER which I made available. Approximately a year later I received a letter from a *** agency which stated I had an unpaid bill for $818 for physician services at Berkeley Medical which later was filed with *** credit bureau as a "derogatory remark".

For your information all my medical bills related to that visit were paid by workmans comp insurance for which evidence can be provided.

I initiated a complaint with *** who did their own investigation and discovered the claim made by the *** agency was fraudulent.

My concern is: It seems that my personal information was fraudulently compromised while I was a patient at Berkeley Medical and at a most vulnerable moment in my life.

I am therefore requesting the Revdex.com to look into any HIPPA violations in Berkeley's practices and contact me.
