Over charging what dental insurance says my portion is. Per my dental insurance, my bills have been paid in full.
I am receiving bills for an additional charge of $***. I paid exactly what the insurance said my portion totaled with exception of the oral hygiene instructions I disputed for $76.00. I paid a total amount of $***. Per the insurance company I owed a total of $*** which includes the $76.00 I disputed and was removed from my charges. I was never told or agreed to higher charges over and above what my insurance approved for my services. They have made zero effort to resolve this issue and say I just need to pay the additional amount.The problem started in December 2018 because they billed my cancelled policy with Blue Shield and not my current policy with Humana. I provided the Humana card in January at my second visit and they continued to bill Blue Shield which obviously was rejected and never updated my account. I have never been treated like this anywhere. They have zero customer service and just send you to collections. It must be a greed driven dental office. I have also requested a detailed statement of my charges and payments.To this day I have not been provided my account statements and details. I requested this again today from a ***, I highly doubt he will email the statement details to me either.I won't hold my breath.
Write the total amount over the contracted insurance rates.

On 12/28/18 I had a root canal performed and a crown was placed only to find out they didn't get all canals.(was in pain for 9m) Had to have it redone
On December 28, 2018, I had a root canal by *** at no time during that visit did he explain to me that he could not get all of the canals. After that visit I went back to see *** in which my permanent crown was placed. The pain from my root canal did not subside and I am still in pain. I scheduled a follow up visit with *** in which I was informed at that time that he could only get half of one of the canals and this could be why I am still having pain. He then went on to say that he could try and go in and fill it but he would have to drill a hole through my permanent crown. I made an appointment in which I cancelled because he informed me this could cause breakage in my permanent crown. This is unacceptable. *** never informed me in my first visit that he couldn't get to all of my canals or I would have waited to have my permanent crown placed. Now nine months later, I am left with the pain and had to find a new endodontist to redo the root canal in which I had completed with Endo Root in Glendale. I want to request a full refund from the office in the amount of $443.20 charged to my Chase account on 12/28/2019, because I had to have it redone. I sent them an emailed complaint on 8/20/2019 on their website page to contact them and no one has returned my call or email. The Endodontist did not provide me with adequate work, I would not have had my permanent crown placed if I had been informed in the first visit that I had a 4th canal that hadn't been treated this caused me 9 months of pain in the tooth.
Since I had to have the root canal redone by a new orthodontist and have to schedule a dentist appointment to either have a new crown or seal placed on the tooth I would like a full refund for the amount which I paid Valencia Dental Group in the amount of $443.20.

As stated in the filed complaint, I would like a refund for the amount I paid to Valencia Dental Group.
I had to have more work done due to the endodontist lack of communication and lack of completing the work.
I have signed a HIPAA form and I have emailed it to the email provided *** and I have also cc'd ***
I would like to have this resolved.

Regarding Valencia Dental Group response, I had went to my original dentist when first experiencing pain within a few months of the root canal because I had an infection which was treated by my primary care physician. In which the symptoms remained from the failed root canal. The dental place where they had to fill the 4th canal that I wasn't aware of due to lack of communication from Dr. at Valencia Dental Group. *** from RootVision Endo informed me that I had an infection and needed the 4th canal filled in which he provided root canal therapy. If they need to verify I was seen with the Endodontist for treatment they can reach the office at ***. Due to having to have treatment redone, I would like to have a refund of what I paid Valencia Dental Group.

they are in my insurance covered plan and I had come to them for check up and in the process they had removed and cut my existing bridge to examine my teeth and I was told they would replace the bridge.
Once the bridge was cut off instead of simply pulling them off as designed I was put in this arkward postion of having no front teeth.
They wanted me to agee to billng/invoice that did not match insurance plan pre-negotiated fees.
They had made pay for tempary bridge when they had no right or reason to cut and destroy my current brideg which I did.
They also wanted me to pay for some crowns in advance even before my teeth were ready for the crowns and before completing the inplants that the caps were meant for.
I explaned to them I did want to pay for the crowns because thier fees did not match what my insurance company pre-negotiated fees on caps but here I am sitting in the dental chair with no front teeth being basically blackmailed into paying.
*** in there office had agreed with me and my wife that since the crowns were 8 months bnefore they were needed that I did not have to pre-pay for the crowns and that would allow me to talk to my insurance company and to work out the issue with the dental office and my insureance company.
I am no learning they are trying to send me to collections for work or crowns that were never rendered. I am requesting your help on his matter.

Billing Adjustment I like Valencia Dental Group to honor the pre-negotiated fees and to adjust my billing and allow m to finish my work as agreed pre my insurance pre-negotiated rates.
