usual and customary lab requirement which we see daily with a [redacted] plan. Our experience with [redacted] plans; as plainly indicated on the patient's insurance card (copy attached); use [redacted]. That is where her specimen was sent. Patients also have a responsibility to provide specific information & requirements concerning their insurance plans. This was not the case on the date of service. We did not hear any disputes from the patient until seven months after these labs were performed.Our doctors and staff strive to provide valuable and compassionate care to all patients. With the above facts stated and in the interest of good will, our office will cover the cost of this lab fee.
