Review: This lab billed for services that were supposed to be free of charge. They billed almost $2000 are are refusing to acknowledge we were even billed despite having records from insurance company.Desired Settlement: Refund to our flexible spending account.

This letter is sent in response to your letter of August 5, 2013, referencing the above- stated ID Number. An exhaustive review of my clients billing files has revealed the following: We are unable to locate any patient in our records by the name of [redacted]. Further, we are unable to locate any billing for any patient for a test performed on May 17, 2013 in the amount of $1,960 or $1,395.89. Finally, Universal Oral Fluid Lab of PA does not offer free testing and would never advise a patient or provider that it does.
