Trellis Chino

Social Worker at Trellis provided false information to hospice in an attempt for a rapid discharge. Social worker advised hospice that a home caregiver was available and scheduled delivery of patient without authorization from patient or family. Incident occurred on a Friday as we attempted to facilitate discharge & contacted other locations for transfer no progress was made due to admission locations closed for business on the weekend. Arrangements for discharge were finally made & we were billed $1,800 fro Trellis Chino, we're disputing these charges as Trellis created the situation that delayed the discharge.
