[redacted] joined Tony's Gym on 3/27/15. At that time, he paid $90 for a Crossfit membership plus a joining fee. Mr. [redacted] signed an agreement that stated this was a month to month open-end membership and if he wanted to cancel, it had to be a 30 day...
written notice signed at the gym. We have Mr. [redacted] signature on the agreement allowing Tony's Gym to debit $80 per month starting 4/20/15 and on the 20th of each month after that until he comes in to cancel. With that signature, he also gave us his billing information to do the debit from. I have gone ahead and cancelled his membership with us so that no other drafts will come out. I will be more than happy to fax Mr. [redacted] his agreement with Tony's Gym, although I have attached it to this email.
