[redacted] is a patient of Dr. L[redacted]'s and came in for orthotics. I do remember her telling us that she was here for a new set of orthotics. Dr. L[redacted] examined her and did all the paperwork for her custom made orthotics. we scanned her with our 3 dimensional scanner as this is a much better way of making...

orthotics. I believe no one uses the casting technique anymore. We have a sign right in the scanning room that there is a 100$ cancellation fee for orthotics. [redacted] canceled many apts. out of 17 apts booked she cancelled 9 of them and missed one. Dr. L[redacted] works 1 week out of every 3 so there would be no way that we would have cancelled a clinic. Dr. L[redacted] tried several times to make them right for her and when the assistant called her to ask if she wanted to try one more time she was verbally abusive to her and we told [redacted] we would offer her the refund of the 469$. Her total was 629$. 60$ for the consultation visit and 569$ for the orthotics that we would keep the 100$ back.
She was sent a check for the 469$ on Monday the 14 th. We will issue her another check for 160$ Aug 16 th.
