FORM.WHEN THE PATIENT CAME BACK FOR HER REPORT OF FINDINGS TO MEET WITH THE DOCTOR TO DISCUSS HER CARE AND TREATMENT PLAN, SHE MET WITH ME, *** *** ***, OFFICE MANAGER

Review: [redacted]'s office received double-payment for services rendered. They were paid at the time of service, but also filed a claim to my insurance company and received FULL payment from them, as well. According to my insurance company, the checks cleared on 2/14, 2/21, and 3/3.

I found out about this from the insurance company. [redacted]'s office never notified us of overpayment. I spoke with the office manager, [redacted], and upon describing the situation, she suddenly said she was very busy and would call me back.

She never did, of course, and now refuses to take my calls. (Whenever someone answers the phone, I give them my name, I get put me on hold, then whoever answered the phone comes back and says that "[redacted] can't take your call now.")

This is unethical business practice at a minimum -- possibly criminal (insurance fraud.)Desired Settlement: I expect an immediate refund of $120.
