We are in receipt of the complaint from [redacted] forpolicy [redacted] Our records indicatethat Mr [redacted] purchased an insurance policy to fund his prearranged funeral contractas selected with Legacy Funeral Chapels on January 28, We mailed a policy and the executed copy ofthe contract to Mr [redacted] on February 4, Only those goods and services included on thecontract that was initialed and signed by Mr [redacted] are covered under thecontract A copy of the executedprearranged funeral contract is enclosed for your records.We confirm that Mr [redacted] did purchase a prearranged funeralcontract with Legacy Funeral Chapels by agent [redacted] Inaddition to the signature of Mr [redacted] and the agent, [redacted] on thecontract you will note that a representative of the funeral home also signed asa party to this contract We offered Mr[redacted] a day free look period when the policy was mailed to him on February 3, After that free look period,per the contract and policy language the cancellation value is the cash valueof the policy funding the contract not a full refund of all premiums paidOnce all requested cancellation documents are received fromMr [redacted] we will process the cancellation of the contract and policy for thecash surrender value of the policy

Once all requested cancellation documents are received fromMr. [redacted] we will process the cancellation of the contract and policy for thecash surrender value of the policy.
