Here is a timeline and documents pertaining to the case. On 10/11/17 I sent a deposit check and signed contract to my Agent *** LIC# *** / Target Insurance Services to start the worker's comp policy with *** Insurance. On 10/13/17 I was sent a confirmation from Target that I was bound effective 10/15/17 with a policy #, welcome packet and certificate of coverage ) On 10/17/17 my check was cashed through ***document provided in attachment). On October 24 a employee of *** fell off a latter on the job and broke his arm. I contacted Target Insurance to file a claim on 10/24/2017 and I was instructed to call *** . At that time *** Insurance informed me that they hadn't issued the policy and was waiting for more information from underwriting.They refused to take information or take info on the clam. I was told I needed to speak to underwriting to file this claim. I called the underwriter multiple times but never received a call back from them. I latter was informed by *** / Target Insurance that the issue was between Targets office and ***, the carrier claimed Target had not sent them payment within 10 days and they had canceled the policy I had never been informed by Target or *** of any needed information or that there was any problem with the policy before this time. We have corresponded by email or phone every day through October, *** my agent had made nothing but excuses and tried to avoid my calls . Finally because of my persistence I received a call from *** Nunez claiming to be one of the owners of Target, she assured me she would get to the bottom of things, that they would find me new insurance policy and resolve the claim problem. I have never herd back from *** and have gotten excuses from *** to date. On November 13 2017 *** referred me to their attorney. I can and will supply Revdex.com with all documentation at their request.
Dear Mr.:
We have been contacted by the Department of Insurance concerning the above referenced complaint. In that regard and as you may recall I am General Counsel for Target Financial and insurance Services (“Target”).
As you may know, I have been in contact with your counsel, Mr. in regards to your complaint and this matter. In that regard we are making every effort to assist you and your counsel towards a favorable resolution of this incident.
Although you retained Target to assist you in placing your Company’s WC Policy, Target communicates only with *** representative, which in this case was, *** Insurance Underwriters. In that regard, and On October 13, 2017 Target received confirmation of coverage from All Insurance with an effective date of 10/1577. Thereafter and in the normal course and handling of such accounts, Target issued a payment 2 days later to All Insurance for this policy. At this point it was expected that the transaction was completed and coverage bound.
Unfortunately, *** is taking the position that payment was not received before the incident and thus refuses to extend coverage. We also don't agree with that position. Since that time Target has made every effort to rectify this matter with both All Insurance and ***. Currently they have refused to comply
