I was scheduled for surgery 12-5-13. I sent the disability paperwork to SOS to complete as it must be submitted to my employer within 15 days of the initial date you are out. I have confirmed after several phone calls that the paperwork was received by SOS on 11-22-13. I received a call from my employer on 12-19-13 that the paperwork was not received and if it was not received by them on 12-26-13 I would not receive my disability payment. I called the SOS office which handles disability paperwork for Dr. [redacted], the representative is [redacted] on 12-20-13. Her voice mail indicates she will return your call within 24 hours, you must leave your date of birth and a reason for your call. I did so.

I received no response, I called again on Monday 12-23-13, left a message on [redacted]'s voice mail. I also called [redacted], a co-worker of [redacted], her number is 703 3476, I left messages for her on 12-23-13, 12-24-13, 12-26 and 12-27. I have yet to receive a return call from either [redacted] or [redacted] regarding my disability paperwork. I also transferred to the operator several times requesting to speak to someone only to be transferred to some other individual and leave a voice mail message. I did receive a call from [redacted] who again confirmed that [redacted] received my disability paperwork on 11-22-13 and [redacted] could not see where [redacted] had submitted the paperwork.

Today I have left a message on [redacted] and Allision's voice to obtain the status as I am not receiving disability pay from my employer due to their error. I have not received a call back from SOS. I was able to leave a message for [redacted] again advising that the disability paperwork had not been completed and my pay has been stopped. No response.
