Dear Revdex.com- due to [redacted] laws we can't elaborate on this matter any further with you. We are in continuous contact with the patient to resolve this matter.
I went to Sunwest to have a crown put on about 6 months ago. I have my permanent tooth, but it has come out twice now. In all my life I have never had a crown come out. Don't know about this office.
Patient was seen our office on 8/28/15, patient was scheduled for a Complete Exam and FMX, we spoke with the patient he confirmed the appointment, he came in a great mood, did not seem to be in any pain at all, he was joking around like he had at his previous appointment, he even was doing card...
tricks with the entire staff, after seeing Dr. [redacted] he needed treatment to be done so we went over the treatment plan with the patient, I even broke the treatment plan down further to make it easier for the patient to see, so patient decided to stay and do same day treatment on his left side. We completed the left side, patient paid his portion and scheduled his next appointment, he left very satisfied and happy, later that afternoon the wife had called asking us what procedures we had done and how it cost so much to work on only one tooth? We did discuss the treatment that we had done and that was it. I believe there was miscommunication between the husband and wife, but we still have the patient on the schedule and not heard from him at all. I did attatch all documents showing patient was presented treatment, he signed and was made aware of all his expenses, and you will also see he filled out all of his health history too. Dr. [redacted] had spoke to him about the health issues also, they discussed the shingles, patient stated he was feeling so much better!
I went to SunWest Dental in Mesa several times and found the dental staff quite helpful. The dentist was indifferent! She addressed me when she first entered the room and after that her conversation was with her assistant, who said very little, as the doctored talked about some guy that she seemed to be very annoyed with! After just a few visits, I realized that I had no desire to continue spending money at SunWest Dental! I would not reccommend this office to anyone.
I have reviewed the response made by the business in reference to complaint ID [redacted], and have determined that this proposed action would not resolve my complaint. For your reference, details of the offer I reviewed appear below.
I have read the response of Sunwest Dental. There are alot of discrepencies with there account They say they have [redacted]s medical histroy, [redacted] had just been released from the hospital in isolation for a blood infection(e coli) on the 12th was done with antibiotics on the 19th but is still suceptable for recurrence as he still has a cathater. Does this not put others at risk. In there notes they speak of an oncologists he is not seeing an oncologist. Also [redacted] has been diagnosed with PHN this is pain caused by the disease of shingles his pain level is at 8 his primary office stated this may never go away they have him on gabapentin with tylenol and hydrocodon to manage pain. We went to primary office on September 2, 5 days after dentist visit with increased pain due to his gums. It appears that the nerves that have been affected by the shingles were agitated by the work done on both sides of his mouth by the dentist(they stated they only worked on left side it showed both sides). Suggestions if pain persists into the 6 month stage to see a nueroligist. This pain has caused in him increased anxiety , his caseworker at the mental health facility are treating him. So there statement that he told them he was doing so much better are not his words. As the PA is not a dentist we were told to go to have [redacted]s mouth checked out to see if there was an infection so we went to [redacted] on September 9 they said the temporary cap on his right side was not there leaving it exposed and he has has severe perodontal disease.
None of the paperwork they sent home with my husband was signed. IN PAPERWORK THAT THEY SENT YOU(Revdex.com) IT SHOWS SOMEONE FORGED MY NAME ON A DOCUMENT I HAVE NEVER BEEN IN THIS OFFICE. I have contacted the bank on this matter. They stated I have not tried to contact them. I have contacted them by registered mail twice.for [redacted]s information. I also contact [redacted] who told me to contact hte [redacted] which I have filed a complaint with them.
So in conclusion untrue statements made about only working on the left side (the crowns they want to replace are on right side of this mouth, that they never sent home signed paperwork and found someone forged my name on paperwork, and the very incomplete paperwork and medical history including my husband is on blood thinners .
