January 28, Revdex.com E Florida Ave Ste Denver CO Claim number - [redacted] To Whom It May Concern: I am writing in response to the above claim number dated 1/13/ In regards to our patient *** [redacted] , she came into our office for Dental treatment and had a balance on her account from previous services Our front office person, [redacted] was told by the patients insurance company, Anthem Blue Cross that the payment for her previous services were mailed to the patient and that is why we asked the patient for the balance in our office [redacted] became upset by this as she stated she did not know of a balance here She elected to not pay for the services and not perform any further dental treatment that day I have called the insurance company today as since her visit we have received some payments from the insurance company Insurance indicates they did not receive one of the claims that is outstanding so I have resubmitted this claim to them I am not sure what the patient balance will be after we receive that payment but I will be willing to work with [redacted] on that when we have closed out the insurance claims I have left a voicemail for [redacted] to go over the above information with her and apologize for the mis understanding between the insurance and our office as well for making her feel we were more interested in the payment then her – that is not the casePlease feel free to contact me at [redacted] or email [redacted] with any further questions or concerns Thank you, [redacted] – Practice Administrator for Stonebrook Family Dental
