To address these concerns, be advised that [redacted] was seen in our office 14 times for various forms of periodontal care. She has limited Oral Hygiene and was informed that without perfect home care, she will initiate further problems. She did have extensive periodontal work in our office on more than one occasion. Since her home care is very poor, it is to no surprise that she is not healing correctly. She went to another dentist who did not see the patient at her original state nor did the dentist evaluate her progress or her home care. He cannot make a judgement on the progress of the case.

I was never given options as to a type of crown, it was picked,a line of credit was immediately opened under my name and the amount of $1900was charged to that credit card - my insurance was not contacted. Attached is abreakdown of benefits in which the amount of $1900 was submitted and insuranceadjusted the amount for the work to $711. I did agree to come in because thestaff claimed that they had contacted my insurance and the price for the workwas final and insurance wouldn't allow for them to charge any less. Afterobtaining more information regarding the agreed upon amount that a dentaloffice within network - which Steinway Dental is - for [redacted] can charge for this work, Icalled back and was once again told that this was the final cost for the crown, eventhough [redacted] representatives have confirmed on more than one occasionthat this is not correct and have directed to me to speak to the dentists’office to have this corrected with them. Their response has been for me to come in and finalize the work and then address the amount I am being charged. I refuse to finalize any work with them and would like a full refund so that I may have this done elsewhere as I'm not comfortable with their business practices.

[redacted] Dear [redacted], Thank you for your letter regarding [redacted] complaint....
On October [redacted] 2014, [redacted] came in to Steinway Family Dental Center for an emergency visit because she had fractured her front tooth. She demanded treatment immediately. Before any treatment was performed, [redacted] was shown crown samples, given all her financial options and informed, of what kind of crown her insurance would cover. [redacted] decided to get an [redacted]rown. She was made aware that her insurance was not going to cover the total cost of the [redacted]rown. [redacted] was given different payment options and decided to finance the procedure with [redacted]. [redacted] paid $1900 for her [redacted]rown and was given a copy of her signed financial agreement. After her treating doctor had prepared the tooth for a crown and sent impression to the laboratory. [redacted] contacted the office and said that she did not want to finish her treatment. The treating doctor explained that her tooth was already cut and that her final crown was at the office ready to be inserted. [redacted] agreed to come back and have her [redacted]rown inserted but failed to show up on two occasions. On Thursday, November [redacted] 2014, our biller spoke with [redacted] and she agreed to come back into the office and complete the treatment as well as go over all insurance information. But again, [redacted] failed to return to the office and failed to return our telephone calls. On November [redacted] 2014, [redacted] contacted our office regarding the above matter and, after careful review, [redacted] was found to be responsible for this bill. [redacted] has agreed on different occasions to complete her treatment knowing that the office already produced her [redacted]rown but has failed to show up or return our calls. If you have any questions or concern please feel free to contact our office. [redacted]

Review: Upon coming in for an emergency crown, I was informed that my insurance would not cover any of the expense for my crown and asked to sign paperwork agreeing to incur the total cost for a crown of $1900. I was advised that a line of credit was being requested through [redacted] Dealer to cover this cost, for work which has still not been completed. This line of credit was apparently approved and charged in full for the $1900 by Steinway Family Dental, for what I was told would be the cost of the crown, due to my insurance not covering any of the cost.
The total cost submitted by Steinway Family Dental to [redacted] for the Porcelain Crown work, which has still not been completed, is $1900. Upon further review of the explanation of benefits received from [redacted] and phone calls to this, my insurance provider, I was made aware that the total amount agreed upon between [redacted] and In-Network providers – which [redacted] happens to be – for a Crown was not to exceed $711, of which the patient is responsible for $355. I have been charged a full $1900 by [redacted] and Steinway Family Dental for work which as per the agreed upon amount through [redacted] should not exceed $711.
I filed a dispute for the cost through [redacted] Dealer, citing that the cost of this procedure has exceeded the agreed upon amount through [redacted] and [redacted] via Steinway Family Dental, as the total cost charged to this line of credit by Steinway Family Dental is a total of $1900. Enclosed is also an Estimate of Benefits sent by [redacted] to myself, explaining the cost breakdown and allowance for the work being billed to insurance carrier by [redacted] via Steinway Family Dental.
I have attempted to get this resolved through Steinway Family Dental and [redacted] to in hopes of coming to a resolution through them with no success. I was told by numerous staff members at Steinway Family Dental, [redacted] included, that this crown was a different type of crown and so it was more expensive and the charge for $1900, processed by Steinway Family Dental through a line of credit opened under my name via [redacted] Dealer, is not reversible and the amount is not subject to change.
I have started making payments to this card because my credit has been threatened and I have been advised that in order for [redacted] Dealer to reverse any part of my balance, Steinway Family Dental will have to refund the charge to me directly.
I have supporting documentation for the a. the costs allowed by [redacted] for this work - $711, b. the actual amount being charged by [redacted] and Steinway Family Dental for this work, c. the dispute submitted, and rejected, through [redacted] Dealer to have the charge corrected or reversed.
Any help to get this issue resolved would be greatly appreciated.Desired Settlement: I am requesting to have the amount of $1900 being charged to me by [redacted] and Steinway Family Dental reversed and the correct amount of $355 charged to the credit card opened by them through [redacted] Dealer.
