have processed thru our 120 day follow-up cycle with phone calls & letters to the patient - with no response and subsequent transfer to the same BD vendor. - Pt is currently making $40 per mo. payments on 2014 acct - she may request consolidation of the 2013 & 2014 accounts from the BD agency. The 2015 account is still with SSM - cannot be combined with BD accounts - the 2015 acct is in the 120 day follow-up process prior to BD -the Pt did call Customer Service on 06/17/15 and requested that we consolidate the 2015 account @ 236.67 balance with the BD accounts. Our Customer Serv rep explained that was not possible. Patient submitted Financial Assistance forms in 2014 but never completed the requests sent between Sept-Nov 2014 for additional required documents. SSM offers consolidation of accounts but only if they are active and not in a Bad Debt status.
