March 31, Revdex.com Dispute Resolution Department NBroadway, Suite StLouis, MO To whom it may concern at the Revdex.com, This is a response to claim number ID [redacted] which was filed against my company, StLouis Physical Therapy, on 3/15/The claim was filed by the husband of a patient we treated at our Ellisville locationThe husband claims his wife was misled regarding the amount she owed for her physical therapy servicesBefore I discuss the resolution that was attempted, I want to clearly explain that each patient before they start their first visit of physical therapy at any of my offices signs off on a “Consent for Care & Treatment” document This document clearly states the patient financial responsibility while receiving care at StLouis Physical TherapyThis involves calling to verify policy specific benefits and providing the details of the insurance benefit quote to the patientI have attached this document for your reviewThe patient’s wife started physical therapy at my Ellisville office on 10/19/The consent is attached and clearly defines the patient payment responsibilityThis consent and explanation of benefits was made clearly to the patientA portion of the explanation of benefits is as follows: “Patient has an individual $deductible of which $has been met and patient will be responsible for the entire cost of PT until deductible is metAs a courtesy we will collect a PARTIAL PAYMENT of $each visit to go towards the deductible until deductible is met...” Regarding resolution, the patient’s husband and I have had 2-conversations on the phoneAfter I spent time reviewing the patient’s consent described above, I told the husband the consent signed by his wife was very clear as to her responsibilityHe then asked me to write 50% off of the bill as he felt his wife was misledRevdex.com, I have been a small business owner for nearly yearsOur company has been built upon its honesty in dealings with patients and people from the medical communityMany times throughout the years, when I felt my staff was not clear on patient financial expectations, I have written off a portion of the patient responsibilityHowever, in this case I am not willing to write off 50% of the patient responsibility; my company did everything right in preparing the paperwork which the patient signed off on before her October 19, visitAfter my last conversation with the patient’s husband it was my professional opinion that no further conversation with the husband would be of any benefitHe told me he wasn’t going to pay the balance in a very angry tone, a tone that professionally and personally I cannot deal withAt that point we turned the patient’s account over to collectionsPlease contact me at [redacted] to discuss this situation furtherSincerely, [redacted] , PhD, PT, DMT, FAAOMPT Owner-StLouis Physical Therapy

First, I want to state that I have never filed a complaint in my entire life on any business in the USAI am very reasonable and if the issue did not bother me this much, I would never file an actual complaintUnfortunately, my wife was mislead by this companyMy wife is originally from Russia, and has been living here for about yearsShe speaks English very well, but to no fault of her own can be too trusting in a company's verbal statements before reviewing and carefully reading the fine print in a consent agreementShe had some neck pains last year, and decided to look into physical therapy and decided to go to StLouis Physical Therapy located in Ellisville, MOMy wife discussed a treatment plan with the associates at this companyThe employee of this company stated MULTIPLE times that the cost per treatment will be $dollars out of pocket and will not have to pay another bill everMy wife asked this employee three times, that it will only cost her $dollars and will

claim was filed by the husband of a patient we treated at our Ellisville location. The husband claims his wife was misled regarding the amount she owed for her physical therapy services. Before I discuss the resolution that was attempted, I want to clearly explain that each patient before they start their first visit of physical therapy at any of my offices signs off on a “Consent for Care & Treatment” document.  This document clearly states the patient financial responsibility while receiving care at St. Louis Physical Therapy. This involves calling to verify policy specific benefits and providing the details of the insurance benefit quote to the patient. I have attached this document for your review. The patient’s wife started physical therapy at my Ellisville office on 10/19/15. The consent is attached and clearly defines the patient payment responsibility. A portion of the explanation of benefits is as follows: “Patient has an individual $5000 deductible of which $389.84 has been met and patient will be responsible for the entire cost of PT until deductible is met. As a courtesy we will collect a PARTIAL PAYMENT of $35 each visit to go towards the deductible until deductible is met...” Regarding resolution, the patient’s husband and I have had 2-3 conversations on the phone. After I spent time reviewing the patient’s consent described above, I told the husband the consent signed by his wife was very clear as to her responsibility. He then asked me to write 50% off of the bill as he felt his wife was misled. Revdex.com, I have been a small business owner for nearly 14 years. Our company has been built upon its honesty in dealings with patients and people from the medical community. Many times throughout the years, when I felt my staff was not clear on patient financial expectations, I have written off a portion of the patient responsibility. However, in this case I am not willing to write off 50% of the patient responsibility; my company did everything right in preparing the paperwork which the patient signed off on before her October 19, 2015 visit. After my last conversation with the patient’s husband it was my professional opinion that no further conversation with the husband would be of any benefit. He told me he wasn’t going to pay the balance in a very angry tone, a tone that professionally and personally I cannot deal with. At that point we turned the patient’s account over to collections.
