Springtree Health Care Center, L.P.

Review: While my mother, [redacted] was a patient at Springtree Health & Rehab Center a caretaker at the facility accidentally dropped her dentures. The dentures broke in half. Springtree agreed since it was their fault they would have the dentures repaired and pay for the repair. The plate was taken to Carilion Clinic Dental Care by a Springtree employee and picked up by their employee then returned to my mother at Springtree after the repair was completed. My mother was not treated or seen by anyone at the Carilion facility.

I continue to receive a bill for this repair for my mother from Carilion Clinic Dental Care. I have been to Springtree with the bill in hand. The bill was taken by their business office manager, Ms [redacted]. She told me the bill had been paid by Springtree. She took the bill and told me to ignore the bill. Ms [redacted] said she would contact the Carilion facility.

I have been in contact with both facilities numerous times over the past few months when this bill

presented itself. I do not want this bill to be outstanding. Please help me with this bill. My mother,

[redacted] did not owe for this repair. She was not seen by the Carilion facility.

My mother has since passed away. I need to have this matter cleared ASAP.

I am requesting the help of the Revdex.com in getting this matter taken care of and I would like written

documentation from both facilities that the balance is settled.Desired Settlement: Documentation that the balance is settled.

Health & Rehab Center by [redacted] has been resolved. The facility paid

the Carilion Dental Clinic on behalf of Ms. [redacted] in May of 2013. Per

documentation that was included in an earlier communication.
