Good afternoon, In response of the complaint filed by * [redacted] We received a written note from patient on a statement that was dated 4-20-2016, the patient disagrees with the charges for the exam and x-ray that was done on 3-9-Mr [redacted] met with our financial coordinator on 3-9-and she went over the proposed treatment, we do not offer free consultations or x-rays and at no time was that relayed to the patient You can see by the attached treatment plan that it was signed by Mr [redacted] and that there is an out of pocket for the consult and x-ray At no time during the consultation did Mr [redacted] mention price or a free consult and he scheduled surgery, he did call and cancel surgery via voice mail on 3-21- We billed Mr [redacted] 's insurance and once they processed the claim we sent a statement to the patient on 3-29-2019, 5-19-2016, and 6-30- At no time did our office hear from Mr [redacted] until his note was received in July and would have been more than happy to discuss his issue with the account balance at any time On 7-26-we did attempt to call Mr [redacted] and before an explanation could be made, he hung up on our staff member I have attached a letter that is dated 7-25-that was mailed to Mr [redacted] , it indicates that we adjusted off the balance as a courtesy There is not a balance on the account and the $has been adjusted off Sincerely, Renee B [redacted] --
