by those we serve. Accordingly, we are anxious to respond
to the above-mentioned complaint and resolve and concerns. However, a more
thorough response to the complaint would require the disclosure of health
information protected by the PHIPP Privacy Rules. Per those rules, we may not
disclose any information without the complaint's HIPAA- complaint
authorization; doing so could subject Spine Institute of Idaho to civil fines.
(See 45 CFR 164.502, 164.508, and 160.400 et seq.). I enclosed a copy of our
standard authorization. Please forward it to the patient to execute and return
