As this issue falls under HIPAA regulations we cannot release any information to anyone.   We have mailed Mr. [redacted] [redacted] a "Consent to Release" form for his signature on 11.07.2017.   As soon as he signs the form and returns the original to us we will be able to respond to you.    Thank you
