Obstetric Financial AgreementThis agreement specifically states the price of the service for an epidural performed by DrLaceyIt also states the patient's insurance may not cover service and if this is the case and the patient chooses to proceed, the patient will be held financially responsible for the billThis is signed and dated by the patient and a witness from our officeA copy is given to the patient and one is included in the patient's chart
As for this service being included in the global OB charge, according to the Current Procedural Coding (CPT) book and the American Congress of Obstetricians and Gynecologists (ACOG) , where the procedure codes originates from (CPT Code 59400, specifically), this includes Fetal Heart Tones, Hospital/Office visits, Initial/subsequent history, physical exams, recording of weight/blood pressure, routine urinalysis, routine prenatal visits, postpartum visit It DOES NOT include epidural anesthesia Also, if this would have been performed and charged by the anesthesia department, the insurance company would have paid the fee The insurance company does not want to pay the physician for doing the service We are willing to give the patient a 20% cash discount
