I made sure to call the parent, apologies for any inconvenience this has caused herI also apologized for the information that was given to her in regards of methods in receiving the form: release of recordsThe form was sent to her via email and received the same dayThe records have been mailed to the address of the dentist she requested and all x-rays email to the email address she providedHer discrepancy has been resolved in the manner she desired and claimant was happy to have it resolved.SincerelyPediatric Manager
