Response:

In response to your letter and the complaint filed by the above mentioned

claimant. I have been out of the office for the last three weeks and was not

able to respond to the patient concerns or your inquiry and sooner.

Mr. [redacted] came in because his crown came off. I have previously made

this crown twice and it has come off each time due to the very small tooth

structure and the decreased retention of the crown. The patient had been

informed of this when the crown was made. I do plan to see Mr. [redacted] on

Tuesday, January 14, 2014 for another appointment. At this time I plan to

take an impression of this tooth and have a lab generated crown made.

Usually lab generated crowns tend to have a higher retention rate because

they have a metal liner. The previous crowns I had made were all porcelain

crowns in order to increase esthetics in this area. I am hoping that the new

lab generated crown will not come off for him. This treatment had worked

in another area of his mouth where we had done another porcelain crown for

him earlier last year.
