Where is she getting the $325.00? Why wasn't I notified of that amount?
This is the Amount owed for previous treatment in that was never paid in full I did send a break down of patient history I wish I could be of more help but when Mrs *** started treatment here I was not the office Manager there are no notes so I do not fully know the agreement they had at the time.. when I added up her payments and what was due the Amount changed from 377..to $in patients Favor

co-pay is $10 per office visit (CDT Code [redacted]) as highlighted in the enclosed [redacted] USA, Description of Benefits and Copayments. In regards to the total amount Ms. [redacted] has been charged, our "Patient History Report" shows that she has been billed for a total of $1,785 since her initial visit on February 20, 2015. To date, she has paid a total of $1,408 and the remaining balance (overdue) is $377. Additionally, under the [redacted] USA (HMO plan) program, co-payments for office visits and/or procedures are collected at the time of service.Hopefuly this helps provide Ms. [redacted] a better understanding of her dental insurance plan and billed charges for services rendered. I disagree they had to see me to add a tooth to my partial plate because they charged me $530.00 dollars for that partial and it was supposed to be $400.00 dollars. I paid rhem $60.00 dollars and that was my final payment. They were paid in full be the episode with my partial. I need proof that they billed my insurance in full for all treatments. [redacted] stated they only billed for one root canal.
