January 27, 2015Attn: [redacted] took residency on April 12, 2014,During his stay for the first 7 months - [redacted] received over 15 write ups which were in violation of his contract.From November 10th to December 10, 2014 - He had received more then 3 write ups within a 30 day...

period.Nov. 10th - For not doing his choreDec 6th  - For leaving personal items in living room, and also another for leaving his dirty laundry in a pile outside of his bed room. (x2)(He would not sign the write up)Dec 7th  - For a dirty room, and also not doing his chore.(x2)I had also given him instructions that he was to pay for his U.a's which were owed for his over night passes , and he was not allowed to take a pass until it was paid.  I let him know he did not have permission to leave for a overnight. Which he did not respond to my instruction's, and he took a overnight pass for the weekend. I had received information that [redacted] had been breaking into coin operated vending machines in the house, and stealing.On December  8, 2014 - I spoke with him and told him that he would need to find another place to  live and to start making other arrangements.December 10, 2014, Again I instructed him that he needed to move out by December 12, 2014.December 11, 2014 [redacted] moved out and had vandalized his room.  which I have a picture of his room - I have all documents included in this response.[redacted]PresidentSerenity House Inc.[redacted]
