11/30/ RE: # [redacted] Dear [redacted] , In regards to ID # [redacted] , this patient was initially seen on 10/31/for New Patient Complete Exam The patient had not worn contact lenses for years The patient was desiring a gas perm multifocal complex lens and this requires a detailed fit and time 11/03/2016: In response to the patient’s request for a contact lens Rx; the patient was informed the doctor had not finalized anything yet because we were waiting to hear back from the patient in regards to her insurance coverage The doctor did NOT do a fitting and had not ordered her any trial lenses 11/04/2016: Patient again called wanting her contact lens Rx faxed to her Again, it was explained a contact lens exam was not done just an overall eye health and glasses exam An explanation was provided in regards to what was involved in a contact lens fitting A contact lens Rx cannot be released until the fit is examined, followed and completed We had not even ordered a lens to evaluate, therefore, no final Rx existed 11/05/2016: The doctor called the patient and left a message that a contact lens Rx had not been fitted or followed so therefore no Rx was written We have to consult with the lab, design the lens and follow it the first time through 11/07/2016: The patient no showed for a scheduled appointment to address this issueThank you [redacted] , O.D., F.A.A.O
