Review: I am writting to you about what syracuse community health center is doing to me. My doctor doctor [redacted] is refusing to fill out my verification form for my medicaid cab for what ever reason I do not know and he wants me to take a city bus witch does not make any sense at all I have health problems with my breathing I am currently on oxygen for that reason and he was the one that had me put on it in the first place and he also knows I cannot walk very far because of my breathing I really do feel im being discriminated against and im very upset what and why he would do this to meDesired Settlement: I would like him to fill out the verification form that medicaid has requested for me to continue to get my cab for my appointment's and I refuse to take a city bus do to my health
