Review: [redacted]I have received periodontal treatment from [redacted]. During the consultation stages on 7-18-2013, she assured me that the entire treatment was covered in the amount we agreed. I asked her during the initial negotiation, that if that was the only payments, I will accept because my insurance policy was going to be terminated mid August. She agreed and stated that my follow-up visits related to the procedure were included in the agreed premium. I made a loan for $3000, with a third party (Care Credit), which she facilitated; my medical insurance paid another portion, and she generously agreed to accept payments on the unpaid balance, she agreed to finance was for $1429. No future services or payments were discussed at the time of the procedure negotiation. After I left the dental clinic, a series of appointment were scheduled in connection with my procedure, included a mid February follow-up appointment, which was included in the procedure. I had partially accepted an offer at mid November and planned to relocate to DC, and I went back to the office to let see if it was possible to do the appointment before my departure. I was referred to set an appointment with the referring dentist (who worked at the same exact location) for a routine cleanup, and [redacted] did ensure that my critical molars, which were the highest focus of the procedure, were untouched. During this visit [redacted] mentioned to the [redacted] hygienist that [redacted] will be revising that unrecovered dental area later. I received a routine clean-up with [redacted] staff in mid December and a schedule my appointment with [redacted] for July (presuming my Departure to DC). Approximately two weeks after negotiations with the new prospective employer, I decided not to relocate to the DC metro area and within days, I contacted the office of [redacted]. Office of [redacted] will send a full copy of dental record to patient. I am willing to resume my monthly payment corresponding to this month as soon as these conditions are accepted.
