HIPAA form is signed by *** ** ***. Consumer appears to be *** ***. I don't have a date of birth for consumer and have multiple patients in our system with that name. I am unable to identify the patient without a date of birth and cannot accept this HIPAA form if the

The supervisor spoke to the patient, [redacted] on 08/29/2016 bout receiving a bill 6 months later. The insurance had previously processed the claim incorrectly, we were waiting for them to reprocess the claim correctly before billing the patient. The insurance reprocessed the claim on 07/29/2016 and a statement went to the patient in August 2016. We apologized for the lateness but wanted it to be processed correctly by the insurance plan, after explaining this process the patients was satisfied. thank you for this opportunity.
