This family had worked with our agency for a little over one year and canceled our services approximately weeks agoFor the particular incident in question, San Antonio Visiting Angels made attempts to reach Mr [redacted] regarding the caregiver's scheduling conflict I personally left Mr [redacted] two voicemails on his listed cell phone number the evening before and morning of the scheduled shift I also attempted to call the home phone number our agency has on file, there was no answer, and I was unable to leave a message there Mr [redacted] denied receiving these messages, however, our agency provide Mr [redacted] with screen shots of these call logs and we are happy to provide the phone records to the Revdex.com as well Since our agency had worked with Mr [redacted] and his mother for over a year, our agency did not send a replacement caregiver because Mr [redacted] has always said no to our offers of back up caregivers both telephonically and through emails since September When I had previously sent over a ficaregiver without receiving verbal approval from Mr [redacted] , he contacted our office demanding to speak with the franchise owner because he was extremely upset that a new caregiver had been sent from our agency without his consent From that point forward, any time our caregiver has needed time off, we have consistently offered to send replacements both over the phone and through email, Mr [redacted] does not accept our offer of back up caregivers and instead cancels the scheduled shifts until their regular caregiver returns Furthermore, our agency attempted to resolve Mr [redacted] concerns amicably by reaching out to him through both the office manager and franchise owner On the date of the incident in question, office manager reached out to Mr [redacted] first and was yelled at, cursed at, and then huon The franchise owner then reached out to Mr [redacted] and was under the impression that the issue was resolved as Mr [redacted] requested that our caregiver return to her shift on Monday 8/14/ On 8/14/17, our caregiver worked her scheduled shift and upon leaving Mr [redacted] sent an email requesting to discontinue services with our agency Franchise owner attempted to reach out to Mr [redacted] at that time, he did not return the phone call Upon receiving the complaint from the Revdex.com, franchise owner again reached out to Mr [redacted] and he has yet to return the phone call

[redacted] two voicemails on his listed cell phone number the evening before and morning of the scheduled shift. I also attempted to call the home phone number our agency has on file, there was no answer, and I was unable to leave a message there. Mr. [redacted] denied receiving these messages, however, our agency provide Mr. [redacted] with screen shots of these call logs and we are happy to provide the phone records to the Revdex.com as well. Since our agency had worked with Mr. [redacted] and his mother for over a year, our agency did not send a replacement caregiver because Mr. [redacted] has always said no to our offers of back up caregivers both telephonically and through emails since September 2016. When I had previously sent over a fill-in caregiver without receiving verbal approval from Mr. [redacted], he contacted our office demanding to speak with the franchise owner because he was extremely upset that a new caregiver had been sent from our agency without his consent. From that point forward, any time our caregiver has needed time off, we have consistently offered to send replacements both over the phone and through email, Mr. [redacted] does not accept our offer of back up caregivers and instead cancels the scheduled shifts until their regular caregiver returns. Furthermore, our agency attempted to resolve Mr. [redacted] concerns amicably by reaching out to him through both the office manager and franchise owner. On the date of the incident in question, office manager reached out to Mr. [redacted] first and was yelled at, cursed at, and then hung-up on. The franchise owner then reached out to Mr. [redacted] and was under the impression that the issue was resolved as Mr. [redacted] requested that our caregiver return to her shift on Monday 8/14/17. On 8/14/17, our caregiver worked her scheduled shift and upon leaving Mr. [redacted] sent an email requesting to discontinue services with our agency. Franchise owner attempted to reach out to Mr. [redacted] at that time, he did not return the phone call. Upon receiving the complaint from the Revdex.com, franchise owner again reached out to Mr. [redacted] and he has yet to return the phone call.
