I first called Salt Lake Dental (SLD) and gave them my insurance information to check if they were in network. They communicated they were in network. After this I went in for a routine cleaning and examine. I was visited by Doctor 1. They told me in this exam that I need to get another procedure done. I scheduled this appointment, after the front desk went over how much my insurance would most likely cover, and how much I would have to cover. During this procedure, it was a different doctor than my first exam. Upon finishing the procedure I paid the amount that was requested. A week later I received and email from SLD saying that my insurance did not cover anything, And that I have a bill due. I then called my insurance to check to see why it was not covered, especially after my first exam was covered by the insurance. They told me that they have several doctors at SLD. Doctor 1 that did my exam was in network that is why my insurance covered it. My insurance company confirmed with me that the Doctor that had done my second procedure was not in network. I then called SLD and tried to explain this situation. The lady I spoke with insisted that I was wrong and that I needed to call the insurance company and figure it out. At this time she did not know that I had already spoke with my insurance, and they had confirmed that they had billed me incorrectly, and or scheduled me with a doctor that is not in network. If the doctor they scheduled me with is not in network, that is SLD negligence. They are my provider, and upon communication that I need to use services that are in network, it is their job to schedule me with the right doctors, and to bill me correctly.

At this point I have called your office two times, and have emailed SLD countless times in attempt to resolve this issue. I will never use this business again, and I would be very wary going here, especially if you are trying to use your insurance. How many other people have been billed incorrectly, and cheated out of their money?

On July 15, 2020 we received an eligibility of benefits from ***s Insurance company United Healthcare for date of service June 29, 2020. Insurance ended up pay $0 for filling. It stated that Alternative benefit applied and $50 yearly deductible was applied. Emailed patient EOB and requesting payment for the remaining balance. United Healthcare sends the patient the SAME eligibility of benefits as they send us. The patient called outraged about receiving a bill. I explained that we were out of network, I didn’t forecast them to downgrade to an amalgam filling and apply the deductible. I didn’t have a crystal ball to know they weren’t going to pay anything. I encouraged the patient to reach out to the insurance company because they dictate the cost. Thats when he started calling me all kinds of derogatory names- STUPID, IT, ***, saying *** You you stupid ***, said I was trying to steal his money. That we should have known they weren’t going to pay anything. He called back later that day after he did follow my instruction of calling the insurance company. He said we processed it out of network Jeni informed him that we haven’t been in network with United Health Care since 2018. As we accept united Health Care we are not in network. That it is the patients responsibility to know benefits. He then demanded that we process as if we were in network. Which is impossible! Patient stated that he was NOT going to pay anything. July 24,2020 (STATE HOLIDAY)Patient emailed stating this was this 3rd attempt. That we billed “incorrectly” out of network. He claimed that there is a different doctor that was in network and he requested that we bill fraudulently under a different doctor. We replied back that he needed to pay his bill. That he was dismissed as a patient at Salt Lake Dental after cursing and calling me the names previously stated above. We billed correctly he saw Dr. Carter who is not in network. There is no dentist at Salt Lake Dental that is in network with United Health Care.He replied Via Email: “There were no curse words used. And even if there was, thats not illegal, and does not excuse your medical negligence.Due to the lack of office willing to take any action to work with me. I will be forced to report yourself and your office to the Revdex.com for negligent medical practices.”

Patient is stating that I am lying about the deductible it is clearly stated on the EOB that the deductible was applied and an alternative benefit was applied.

Like I said in my previous message. Before I scheduled my appointment, which was at your front desk. I asked the lady who scheduled my appointment to make sure it was in network. Not on the phone.

We record all our calls. I pulled up the call on June 22, 2020 @ 10:12 am. Patient never asked if we were “IN NETWORK.” As per the recording Patient said: “I have dental insurance, do you guys check to see if that works with you guys?” “What is is it?” “It is United Health Care.” “Yes we do take it. I’ll take your name and address and I’ll take your united Health care info.”I have downloaded via Movie if the Revdex.com would like it for further investigation.We told the patient that we take United Health Care. Patients are responsible to know what his or her insurance company policy benefits are we estimate copayments but can not guarantee what the insurance company is going to pay. The insurance sent the eligibility of benefits to the office and to the patient. The patient owes what the insurance states. It reflexes that the deductible was applied and they downgraded the filling to amalgam. This is a simple case of a patient not wanting to take responsibility and pay a bill.
