Review: I had a routine visit to Dr. Shore's office on May 8, 2012. The office visit total was $140. I paid a co-pay of $30. The office manager claims my health insurance was denied and sent me a bill for $110. The bill has been sent to a collections agency; this has been reported to credit report and will remain there until February 2019. I moved from Maryland to Massachusetts on May 15, 2012. I submitted a change of address to Dr. Shore's office on October 2012 for [redacted], VA [redacted]. In February 2013 I sent a change of address to Dr. Shore's office for the address at [redacted] MA [redacted]. To date, I have not received one bill from Dr. Shore's office to my Massachusetts address.Today I called my [redacted], my health insurance company at the time of my 5/8/12 visit to Dr. Shore. The representative said a claim by Dr. Shore had never been filed with [redacted]. I called Dr. Shore's office today and spoke with [redacted], the office manager. She says she updated the billing company with my COA on 10/9/12. I spoke with [redacted] at the billing company and she said she received a COA from Dr. Shore's office on 3/13/13. [redacted] also said the claim had been filed with [redacted]. [redacted] says there is no record of my [redacted] in my medical record and that maybe I forgot to inform the office secretary of my change in insurance. I replied that every visit to Dr. Shore's office the secretary verifies my demographic information - address, insurance, PCP - and makes a copy of my insurance card. It was no different on my 5/8/12 visit.Desired Settlement: I'd like Dr. Shore to file my claim with the correct insurance company. I would also like the charge removed from my credit report. If, upon filing my office visit with my correct insurance provider, I owe more than the $30.00 I paid at the time of my visit, I will do so.
