My office responded with fax to [redacted] on 3/3/2016 and again on 319/2016 with request for a 25.00 charge for copy of office records. The copy is customary. We have not had a response from the legal group or Mr. [redacted]. Additionally, records have been sent in the past to Mr. Hussar's previous legal representatives. Mr. [redacted] has not ever personally contacted this office regarding processing of records as described in his complaint. Itemized statements have been sent to [redacted] and Ann [redacted], there legal representatives, [redacted], LLC, [redacted] Insurance Company and [redacted] BC/BS. Response from insurance companies are generally include an EOB (Explanation of Benefits). The statement that he may be referring to, that he states was incomplete, is most likely form the collection agency who is attempting to collect balance due from 2011.Thank you.
