signed for this refund.She had a routine eye exam on March 27, 2015 with Dr. [redacted] of which she had no complaints. She did use herbenefit for services rendered on that day. A prescription was generated and she is welcomed to take this to anotherOphthalmology/Optometric practice to have filled.May 15, 2015:1 spoke with [redacted] today on the phone and we had a very pleasant conversation. She has a copy of hersigned receipt for the refund so she declined my offer to send her another copy. She reports having a greatexperience with Dr. [redacted] and had no complaints about that visit. She took her prescription to another locationand had it filled there. She said she is currently happy with the glasses she now has. She also said that it had beenawhile since she's had glasses at that could have been part of her issue when she was here. I apologized for herexperience here and she was thankful that I called her.
